What to Bring to Your Iowa Permit Test

Proof of identity and date of birth
A valid, unexpired U.S. passport or passport card works, or a certified copy of a birth certificate issued by a state vital statistics office. Iowa Administrative Code 761-601.5 spells out that every applicant must prove identity, date of birth, Social Security number, Iowa residency, and lawful presence in the United States.
The manual itself is light on this specific checklist -- it only lists the broader categories (vision screening, knowledge test, driver's ed if under 18, driving test) rather than the identity-document list -- so double-checking current requirements at iowadot.gov before the appointment is worth the extra step.
Two residency documents, not one
Iowa requires two documents showing your name and current Iowa street address -- utility bills and bank statements are common examples. A P.O. box address doesn't satisfy this requirement; it needs to be a real street address.
Because two separate documents are needed (not one), it's worth confirming both are current and display a matching name before the appointment, rather than assuming a single strong document (like a lease) is sufficient on its own.
Proof of Social Security number
An original Social Security card is the simplest option. If it's not available, a W-2, a Social Security Administration 1099 form, or a pay stub showing the applicant's full name and complete SSN can substitute.
This document requirement applies regardless of age -- a 14-year-old applying for an instruction permit needs the same SSN proof as an adult applicant, just paired with additional parent-consent paperwork on top.
For applicants under 18: parent or guardian consent
A parent or guardian must provide written consent for an applicant under 18, either by appearing in person at the DOT service center or by completing Iowa's Parent's/Guardian's Written Consent Form (form 430018) and bringing it along. This isn't optional paperwork -- without it, the permit application can't proceed for a minor.
Because this requires either a parent's physical presence or a specific completed form, it's worth confirming which approach a family plans to use before the appointment day, rather than assuming a phone call or verbal confirmation from a parent will suffice.
Before you go: confirm the appointment and location
Iowa's manual recommends scheduling an appointment at a DOT service center via iowadot.gov/service-selector, or checking with a County Treasurer's Office directly about how they handle walk-ins, since procedures vary by location.
Not every service, including some permit-related transactions, is available at every location -- confirming the specific office handles instruction permit applications before making the trip avoids a wasted visit.
