What to Bring to Your Nevada Permit Test

Proof of identity and date of birth -- from a specific list
Every applicant needs an unexpired original or certified copy of a document proving full legal name and date of birth: a state-issued birth certificate, a valid U.S. passport or passport card, a Consular Report of Birth Abroad, or -- for those born outside the U.S. or holding permanent resident status -- the applicable naturalization, citizenship, or permanent resident documentation.
Documents not accepted include a hospital-issued birth certificate, any out-of-state driver license or ID card, a foreign birth certificate, a border crossing card, or a consular identification card -- worth checking against this exclusion list before assuming a document will work.
Proof of Social Security number
Applicants must present a Social Security card, a W-2 with the full SSN, an IRS Form 1099, or a paystub with the applicant's full legal name -- handwritten paystubs are explicitly not accepted. All documents must be originals containing the complete Social Security number, not a partial or masked version.
A Driver Authorization Card applicant, notably, doesn't need a Social Security Number at all -- the DAC path exists specifically for applicants who can't provide one, though it comes with its own separate document list.
For Real ID specifically: two residency documents
Applicants seeking a Real ID-compliant license or ID card need two documents proving Nevada residency, each displaying the applicant's correct residential address -- utility bills, mortgage statements, rental agreements, and bank statements are all acceptable examples.
A standard (non-Real ID) license doesn't require this residency documentation, but it will be marked "NOT FOR REAL ID PURPOSES" and won't be accepted for boarding a domestic flight or entering certain federal facilities -- worth deciding upfront which type of card is actually needed.
For applicants under 18: school and driver-education paperwork
A minor applying for an instruction permit must provide DMV Form DLD-301, the Certification of Attendance, signed by a school principal or designated official confirming minimum attendance standards -- and the form must be signed and dated within the previous 60 days of submission. A parent, guardian, or other authorized adult must also co-sign the application itself.
When applying for the actual license later, the same minor needs a Certificate of Completion for driver's education and a completed Beginning Driver Experience Log (DMV Form DLD-130), filled out in blue or black ink documenting the dates and times of behind-the-wheel practice as they happened -- not reconstructed from memory at the last minute.
For the skills test specifically: a roadworthy vehicle
The vehicle used for the driving skills test needs current registration and license plates, an evidence-of-insurance card, and working headlights, taillights, brake lights, turn signals, horn, seat belts, tires, and windshield wipers -- the examiner checks this list before the test begins, not after.
Passengers and animals aren't allowed in the vehicle during the test, and using a cell phone during the drive results in automatic failure -- along with using driver-assistance features like parking assist or cruise control, which aren't permitted during the evaluation.
