What to Bring to Your Oklahoma Permit Test

Proof of identity comes first

Service Oklahoma requires an original or certified primary identification document, and it has to match the full legal name that will appear on your license. Acceptable documents include a certified U.S. birth certificate, an unexpired U.S. Passport, or an Oklahoma license or ID card originally issued after November 1, 2007.

Notarized documents or photocopies are explicitly not acceptable -- you need the real, original or certified version, and if you're not a U.S. citizen you'll need additional documentation showing you're in the country legally.

Proof of your Social Security number

If you've been issued a Social Security Number, you need to know it and provide it when you apply for an Oklahoma driver license or ID card. The number itself won't actually appear anywhere on the license.

This is a separate requirement from your primary ID document, so bringing your Social Security card (or otherwise confirming the number) alongside your birth certificate or passport is worth doing before you head in.

Driver education proof, if it applies to you

If you're applying at 15, you'll need to show a completed driver education certificate, a contract with a commercial driving school, or a notarized driver education form from a public school -- or, for the Parent-Taught program, the SOK-approved Parent-Taught Affidavit and a purchase receipt for the course.

The manual also notes this cuts both ways for anyone under 18 generally: if proof of driver education completion isn't presented when it's expected, additional restrictions may be imposed on the license.

Insurance proof -- but only if you're taking the driving test

You only need to bring proof of auto liability insurance on the day you're actually taking the driving test, not for a written-test-only visit. The vehicle you use for the test also needs a current insurance verification form that can't exclude you as a driver.

If you have a health or physical condition that might affect your driving ability, you may also be asked for a doctor's statement about that condition before you're allowed to test.

What the vehicle itself needs to pass

Before the driving test, Service Oklahoma's own pre-test checklist requires a valid license plate, working headlights/taillights/brake lights/turn signals, a functioning horn, a proper exhaust system, and tires with legal tread depth.

The windshield also has a specific rule: it can't have cracks or chips in your line of sight, and any tint can't extend more than 5 inches from the AS-1 line on the windshield.

Fees you'll owe once you pass

You don't pay anything up front just to take the tests. Once you've passed everything required, the examiner collects the fees -- a Class D Learner Permit or license totals $42.50 -- and issues a temporary paper license while the plastic card arrives by mail.

If you end up needing a retest, that's a separate $4 fee each time, so it's worth showing up prepared rather than treating the appointment as a low-stakes dry run.
