What to Bring to Your Colorado Permit Test Appointment

The identity and residency documents every applicant needs

Every first-time applicant, regardless of age, needs to establish who they are and where they live before a Colorado driver license office will move forward with a permit application. That means a proof-of-identity document (a certified birth certificate or a valid passport are the most common choices), documentation of a Social Security number, and two separate proofs of a Colorado residential address, such as a utility bill or bank statement. Because identification requirements can vary slightly by document type and citizenship status, it's worth double-checking the DMV's current document list before the appointment rather than assuming last year's requirements still apply exactly.

If the knowledge test was completed online through the @HOME platform rather than in person, that online-test completion should already be on file electronically and doesn't typically need a printed proof -- but if the written test was instead completed through a driver education school, the applicant should bring that school's Driving Knowledge Test completion form to the appointment as backup documentation.

Extra paperwork specifically for applicants under 18

A minor's application can't be completed with identity and residency documents alone. It also has to be accompanied by a signed Affidavit of Liability and Guardianship (DR2460), and that affidavit specifically has to be signed in front of a driver license office employee or a notary public -- meaning the parent, stepparent, guardian, or other responsible adult accepting liability generally needs to either come to the appointment in person or have the document notarized in advance. Minors in foster care are exempt from this particular requirement under certain conditions.

Applicants younger than 16 years and 6 months should also be ready to show proof of their 6 hours of behind-the-wheel training with an approved instructor (or documentation of the substitute parent/guardian hours, where that applies). Bringing incomplete paperwork is one of the most common reasons a permit appointment has to be rescheduled, so it's worth confirming every document is in hand -- not just planned for -- before heading to the office.

The fee to actually bring with you

Documents alone don't complete the appointment -- the instruction permit itself carries a $19.00 fee under the Colorado DMV's fee schedule (raised from $18.52 effective June 30, 2025). That fee is separate from anything already paid for a knowledge test attempt, whether that test was taken in person or through the @HOME online platform.

Anyone who tested through @HOME and is now coming in to pick up the physical permit should bring proof of that completed online test result along with the standard identity and residency documents -- the appointment to collect the permit is still an in-person step even when the knowledge test itself wasn't.
