What to Bring to Your LA Permit Test

The documents Louisiana's application requires
First-time applicants must provide proof of identity, evidence of lawful status in the United States, date of birth, a Social Security number, and at least two documents demonstrating their principal residence address. These are the baseline documentation requirements for any Louisiana driver's license or permit application, not something specific to just the Learner's Permit stage.
If you're upgrading an existing Learner's Permit to an Intermediate license at 16, bring the signed parent or guardian statement confirming at least 50 hours of behind-the-wheel driving experience, including at least 15 hours at night -- without it, the upgrade application isn't complete.
The knowledge test itself
Louisiana's knowledge test is given on a computer in most offices and draws its multiple-choice questions from the information in the Driver's Guide. You need to answer 80 percent of the questions correctly to pass, and proper identification is required every time the test is taken -- including on a retake.
The Guide's own test-taking tips are worth following literally: read through the Driver's Guide multiple times beforehand, focus on memorizing numeric details like distances and hour requirements, and arrive at the licensing office at least an hour before the last available test time that day.
The vision screening
Your eyes are screened when you apply for or renew a license or Learner's Permit. This isn't a full medical exam -- it's a quick check confirming your vision meets a minimum standard of 20/40, corrected or uncorrected.
If you fail the vision screening, you'll be given a vision report rather than an outright denial; follow-up with an eye care professional is the next step before OMV can proceed with your application.
If you're a minor: who needs to come with you
For a minor applying for a driver's license, a custodial or domiciliary parent, tutor, or legal guardian must accompany the applicant, present their own identification, sign the application, and provide proof that the vehicle used for the road skills test is insured.
This accompaniment requirement is specific to the licensing application and road skills test, separate from the day-to-day supervision rules that govern who can ride along while a permit holder practices driving.
What you won't need on knowledge-test day
The knowledge test itself doesn't require bringing a vehicle or proof of vehicle insurance -- that documentation only becomes relevant later, at the road skills (driving) test appointment, where you'll need a vehicle in safe condition with a valid inspection sticker, current plate, and registration.
Confusing the knowledge-test checklist with the later road-skills checklist is an easy first-time mistake to avoid: the knowledge test is entirely about identity, residency, and lawful-status documents plus the vision screening, not anything to do with a vehicle.
