What to Bring to Your Utah Permit Test

Schedule an appointment first -- walk-ins wait longer
Utah's Driver License Division recommends scheduling an appointment for all services through dld.utah.gov, specifically to reduce wait time. Walk-in service is still available, but walk-ins may experience much longer waits, since scheduled appointments take priority in the queue.
The appointment scheduler is also where an applicant selects the correct license type and service before showing up -- scheduling under the wrong service category (for example, a driving skills test booked under a renewal appointment) can mean the appointment isn't fulfilled as expected and needs to be rescheduled.
Proof of identity and lawful status -- pick one from a specific list
Every applicant needs one document proving identity and lawful status: a valid, unexpired U.S. passport or passport card; a certified copy of a birth certificate filed with a State Office of Vital Statistics (hospital-issued, miniature, or laminated certificates are not accepted); a Consular Report of Birth Abroad; a valid Permanent Resident Card; or a Certificate of Naturalization or Citizenship.
Documents have to be originals or certified copies from the issuing agency -- faxed or photocopied documents are explicitly rejected. All submitted documents are imaged, stored securely, and returned to the applicant the same day.
Proof of Social Security number -- also one from a specific list
Alongside identity proof, an applicant needs one document verifying their Social Security number: a signed Social Security card (an unsigned stub isn't accepted), or -- if the card itself isn't available -- a W-2 form, an SSA-1099 form, a non-SSA-1099 form, or a pay stub that shows the applicant's name and full Social Security number.
Applicants who are lawfully present but ineligible for a Social Security number can instead bring a letter from the Social Security Administration confirming that ineligibility, which substitutes for the standard SSN documents.
Two residency documents, each dated within 90 days
Utah requires two separate documents proving Utah residency and mailing address, each displaying the applicant's name and address and dated within the last 90 days: acceptable options include a bank statement, court documents, a current mortgage or rental contract, a major credit card bill, a property tax notice, a school transcript, a utility bill, or a vehicle title.
Because the 90-day window is enforced strictly, it's worth pulling fresh copies of these documents close to the actual appointment date rather than reusing paperwork gathered weeks earlier for an unrelated purpose -- a document that was current when first collected can still age out of the window by appointment day.
For a first-time license (not just a permit): proof of driving experience
For a first Utah driver license specifically -- this isn't required just to get a learner permit or an ID card -- an applicant also needs either evidence of completing a driver education course (if under 19) or evidence of a driving privilege already held in another state or country.
This document sits on top of the identity, Social Security, and residency requirements above; skipping it at the permit stage is fine, but it becomes a genuine requirement by the time an applicant is ready to convert a permit into an actual license.
Payment and what to expect once you're there
Fees can be paid by check, credit card, or cash, but checks must be made payable to the Department of Public Safety in the correct amount -- payroll or two-party checks are not accepted, and a returned check adds a $20 service charge to keep driving privileges active. Bringing the confirmation number for a scheduled appointment is also worth double-checking before leaving home.
Arriving on time matters beyond courtesy: the office cannot guarantee completing all necessary processing, including the written knowledge test, after closing time, and a driving skills test may require an entirely separate appointment regardless of how the visit for the written test goes.
