A certified birth certificate and signed Social Security card, every time
The handbook is specific: each time you apply and test for a Level 1 GDL Instruction Permit, you must show a certified birth certificate issued by a state or governmental vital-records agency, and a Social Security card signed by you. This isn't a one-time requirement satisfied at your very first visit -- it's checked at each application and test.
The same dual requirement applies again when you apply and test for the Level 2 Intermediate Driver's License, alongside your 50-hour driving log and all other licensing documentation.
Parental or guardian consent, unless you're married
For a Level 1 GDL Instruction Permit, you'll need written parental or legal guardian consent on form DMV-DS-23P. The one exception is a married applicant under 18, who instead needs to present a certified copy of their marriage certificate rather than parental consent.
Because this form has to be completed and brought with you rather than signed on-site from memory, coordinate with your parent or guardian well before your appointment date.
No electronic devices allowed in the exam room
The handbook repeats this rule at every GDL stage: no cell phones, PDAs, Bluetooth devices, or other electronic devices are permitted in the exam room, and only the applicant taking the test may be present. Leave devices in the car or with whoever drove you.
This applies uniformly whether you're testing for a Level 1 GDL permit, a Level 2 Intermediate License, or a standard Regular Instruction Permit at 18 or older.
For a Level 2 license specifically, bring your supervised-driving proof
Moving from Level 1 to Level 2 requires either a parent/guardian-certified log of 50 hours behind the wheel (including at least 10 at night) or completion of an approved driver's education course. Whichever path you took, bring the documentation with you -- it's checked alongside your identity and residency documents.
You'll also need proof you've completed 180 consecutive conviction-free days at Level 1 immediately before applying -- this is verified against your driving record, not something you need to produce separately, but it's worth confirming your own record is clean before scheduling the appointment.
Budgeting for the fees
West Virginia's knowledge test costs $7.50 per attempt, and a Level 1 GDL Instruction Permit itself costs a separate $7.50. If you need a copy of your driving record later, that's available online for $12.50 through DMV Online Services.
Because a failed knowledge test means paying the $7.50 fee again for your next attempt, it's worth preparing seriously rather than treating a first try as a free practice run.
